2004 Annual Review
Strategic Plan Survey

Dear friends of Elm City Center (ECC)

 

We are doing an annual review of how well ECC did in 2003 in meeting it’s goals and providing services to people with disabilities. The survey asks you to rates our services and do a SWOT (strengths, weakness, opportunities and threats) analysis for ECC.  There are no right or wrong answers and we value your input! The results of the survey will be published in our quarterly newsletter.  Since you are on the Internet here is how you can fill this form out:

 

Step 1. Copy all of text in this memo into you word processor. If you do not know how follow these steps:

Move your cursor to the "E" in Elm on the first line. Left click your mouse, hold down the left button, and take the cursor to the bottom of the page. You will see the line that talks about a return date. The text will have changed color. You do not need the text in the headers above this memo or the column at the far left. When at the end take your finger off the left button and move the cursor up to the "Edit" button on your browser. Should be in the upper left corner. Click on "Edit" and click on the "Copy" option that will appear.

Turn on your word processor. In Windows that means clicking on "Start" in the lower left corner and then use the menu to open whatever word processor you use. Once the program opens click on "File" and then click "New". A new file will start. Simply place the cursor in the file and press "Ctrl-V" or click "Edit" then click on "Paste" and this file will appear in your word processor file. Formatting may be slightly off but that is easy to fix.  

Step 2. Edit  the form by typing in your information. Do not add graphics or any special formatting commands. We use Word and WordPerfect so we should be able to read your message. Each section of this form is in it's own "box" so you will not run out of space.

 

Step 3. Save your file like you always do.

 

Step 4. Once you are done, you have 4 ways of getting it back to us. All these ways work.

O Print a copy and mail it to or drop it off at Elm City Center, 1314 W Walnut, Jacksonville, IL 62650.  
O Print a copy and fax it to Elm City Center at 217-245-2350  
O Send it as text email. Copy the completed edited file with your word processor. Open your browser and get into the email section where you can send a new file. Paste the file into the message section. Our email address is ecc@elmcity.org. Put "ECC 2004 Review" as the subject. Click on "Send"  
O Send your file as an attachment to email. Get to the email section of the browser. Fill in the email address and subject lines with the above information. Click on the attachment button. Each browser is slightly different, but attachment buttons often have a paperclip picture on them. Go find your file and simply click on the file name. The file name should appear in the email form.  Click on "Send" and the file will automatically come to us attached to your email  

A summary of all the responses we receive will be published in our quarterly newsletter and on our webpage. We will not release your name, but may call you if you volunteer to be part of our focus group. Details are at the end of this memo.
